Rural Health Transformation Project (RHTP)
The Rural Health Transformation Project (RHTP) is a $5 billion federal initiative designed to support long-term transformation of rural healthcare delivery systems, with a focus on advancing value-based care, improving access, and strengthening outcomes in rural communities. Illinois has been awarded approximately $193 million annually for five years, representing a significant opportunity to invest in the sustainability and future of rural healthcare.
ICAHN has been identified in the State's application as a key implementation partner and is expected to administer approximately $50 million each year across three primary initiatives:

Technology Transformation Initiative
infrastructure, cybersecurity, data, and innovation

Hospital Workforce Investment Program

Chronic Disease and Population Health Funding
These responsibilities align closely with ICAHN’s mission and established capabilities while also representing a meaningful expansion in the organization’s scope, scale and visibility.
At this time, several critical implementation details remain unresolved, including:

State appropriation and funding timelines

Subaward structures and eligibility criteria

Administrative funding to support program oversight
As a result, ICAHN’s role is defined in principle but still evolving in execution.
